A use of multi-field formula?

Hello Alteryx community, any tips to use the multi-field formula (or any other way) so create the output like below, please?

You could try something like this.  Would be able to run every week without any type of update to the workflow.  Attached is the workflow.

Ingenious, thanks @Emmanuel_G  .. I learned a new way to use the multi-row tool :-)

Ingenious, thanks @Brian32   .. I learned how to do this without the multi-row/field tool. Also being able to this in "auto" fashion is an icing on the cake :-)
