run workflow with list selection

Question

Hi - is it possible to create filtering criteria with list(cutomer zipcode), check box (male/female), drop-down list(preferences) etc. and based on the selection run a workflow and macro to create a report of let us say customers not contacted recently. I know how to create a workflow and batch macro, however struggling to create a splash page that marketing can use based on their preferences provided in various filters.
