XLookUp (Multi-Criteria) and Restructure Data

This is a two-part question:

1. In the picture above, I created a XLookup in Excel to find if a row said yes and made it return the different options available (e.g. single, engaged, married). Is it possible to do this in Alteryx? I have 96 questions with 3 - 6 choices that go across rows 2 and 3, with 5K responses, which make up the rows. There are a few questions where the user can select two choices.

 

2. Underneath the question (in the above picture) it says, "Score". Under that it has a "-1". The options are pulling in as F46, F47, etc (I assume because the cell is blank). Once I combine the choices in the XLookup column, I need to transfer it to a new sheet, so it looks like the below picture:

Is it possible to do this through Alteryx? I have been playing around with the tools, but have come up empty. Any guidance is appreciated! Thanks in advance.

There is a KB article that goes over how to do Xlookups in Alteryx, which would be a great place to start!

 

If you can provide a copy of your workflow, and some sample data, I'm sure the Community can also provide you with alternative ways to achieve this as well!
