Using header from different rows in Excel to form new column in Alteryx

Trying to utilize Alteryx to reformat an Excel document for PowerBI and running into an issue where I need headers in excel to be in a different column.

Just wondering how I would do this in Alteryx.

My input/desired output is attached in the excel sheet.
