I have Data Like below

Col A	Col B	Year	JAN	FEB	MAR	APR	MAY
Chair	Office	2021	10	20	30	40	50
Chair	Office	2022	30	40	50	60	70
Pencil	Library	2021	15	25	35	45	55
Pencil	Library	2022	25	35	45	55	65

And I want the output like below

Col A	Col B	Date	Value
Chair	Office	Jan-2021	10
Chair	Office	Feb-2021	20
Chair	Office	Mar-2021	30
Chair	Office	Apr-2021	40
Chair	Office	Mar-2021	50
Chair	Office	May-2021	60

 help me in achieving this

Data Like this ,

Need Output like this.

Name	date	100	101	102
suresh	Jan-21	1	2	4
suresh	Feb-21	4	5	6
