Transpose selected rows to columns

Hi, would like to seek for guidance on how to transpose from input to desired output from the attached workflow. Please help. Thank you!

Is this data very similar to your actual data?   Can you identify the values that go in the different columns by keying off a string that starts with "ID_" or "NUM"?

If not, how would you manually identify which original Name value goes in the new columns for Name or Name2?
