Transpose data to create a date colum

Hope you are well. I need help. Currently my data is in the attached file "input" and the desired "output"

I would like to have date as the separate column with year and month. or both year and month in same column

Thank you so much. It really helps. Can you please help me with one more thing. I am trying to replicate something like below using your workflow.

I would like to have one column "Date" with all the values in front of them. However in the below its creating four column. how to modify the workflow please?
