Transformation help needed

And I want to convert this data to below format -

Using Alteryx how can I achieve that? Need help.

I've attached my initial thoughts on getting it in that format. One thing to note would be Alteryx works on a table format and the structure you want has columns at different levels which isn't a good format for tables.  

I would also point out its a lot easier to help with problems like this if you say specifically what your struggling with rather then just post the entire peace of work for someone else to do.

Thanks for your help, but it's not the entire piece of data. I just wanted to know how to transform from source to destination format.
