Building on the work of @DavidP(who did all the heavy lifting), here's a version that breaks out the column headers into 3 separate rows.

This is the most that I could do. Since, it can't have more than one set of headers, you can use the first two columns as your headers while taking the output in whichever visualisation tool that you're using.

This is how I would approach arranging the columns - you then just have to tidy up your column names.
