Split Data including Header in .csv file

Hi Everyone, i need help from you as i am a newbie in Alteryx. 

i have set of data in .csv file and the data in separated by semicolon. Now i want to separate the data into few column shown as picture. Hope someone is willing to guide me.

The raw data is shown is Excel Sheet 1

the expected output is shown in Excel Sheet 2

Attached also the Excel File contain data in sheet 1 for you to do testing.
