Specifying the output file name in analytic app and adding image to analytic app

I have a workflow with the following analytic app interface. Now, I want the specify the file name or change the change 'file save location' so that the user knows which fie to save at what location.

As of now, I am using a simple output file tool with a drop-down to do this.

Also, how to add an image to the below interface (maybe of the left-hand side).

I did this through the interface designer.
