Some one Please help me with formula: How to create If condition when we have 3 rules

Hi, 
I need output like this in SIRVA/ILH No Relo Column:

Situation 1:
If there is date in SIRVA Date then SIRVA/ILH No Relo Report="SIRVA DATE"

Situation 2:
If there is date in ILH No Relo Report then SIRVA/ILH No Relo Report="ILH No Relo DATE"

Situation 3:
If there is date in both SIRVA Date & ILH No Relo Report then SIRVA/ILH No Relo Report="ILH No Relo DATE & SIRVA"

Try the formula below. I've played around with the logic as it was triggering incorrectly:

 

IF !isempty([SIRVA Date]) AND !isempty([ILH No Relo Date]) 
THEN "ILH No Relo Date & SIRVA"
ELSEIF !isempty([ILH No Relo Date]) 
THEN "ILH No Relo"
ELSEIF !isempty([SIRVA Date]) 
THEN "SIRVA"
ELSE "N/A"
ENDIF
