Salesforce Output Tool

This morning, I was unable to run a version of the Salesforce Output Tool that I have been using for months.  Apparently, it has been detracted.

 

After installing the newer version, I am unsure how to use it.  The previous version had very clear instructions (where to put username, password, security token).  This version is similar to the DCM inputs...any insight on how to actually connect to Salesforce?
