Rename column name with adding date from another column

Hi Team, 

I have been trying to rename a column name using Dynamic rename however not able to, since Replace function doesn't work dynamically as date column will keep on changing as old data will be replaced with the newer one and I want to add the Date from Date column to the first two column names:
