Putting Values From Multiple Rows & Columns into One Single Column

I have data in a format similar to this:

 

 ABC
D###
E###
F###

 

 

I have been able to put A, B, and C under a new column named "Entity" and D, E, and F under a new column named "Description", but I am having trouble putting all of the values into their own column. This is what my expected outcome is:

 

EntityDescriptionAmount
AD#
BD#
CD#
AE#
BE#
CE#
AF#
BF#
CF#

 

Any help is very much appreciated!

This is pretty straight-forward and a use of the TRANSPOSE tool (using a group by function):
