Any idea what causes this not to happen? Note even when the tool underneath the annotation is selected it's still obscured. This doesn't always happen but I'd like it never to happen.

You can change the layer of individual tools by right-clicking on the tool and selecting "Bring to Front".
Otherwise you can turn off tool annotations in the Canvas options of the Workflow Configuration window and not deal with it at all.
