What's the issue you're having? I am uploading a sample workflow, but please provide more specification as to what you're requirements are, if necessary.
@coryanthony I think this is what you're looking for. If you want to concatenate a field and not have duplicate values in it, you need to get rid of duplicates. I used a Summarize tool to do it. Let me know how it works for you.
