Parse PDF form with Radio buttons

Hi,

 

  I am trying to parse a PDF form which collects responses from patients for a multiple-choice questionnaire. In the PDF form, there are multiple sections with questions under each section. In each section, the response options are displayed at the top, and there are radio buttons next to each question to select an option for each question.  When I use PDF to text , I am not able to see any data related to the data entered in radio buttons. Can anyone suggest if there is a way to make this work.

Many things can go wrong, without you sharing more data is will be hard to guess what goes wrong, can can be tool setting, but hard to say without seeing it
