Output issues -

I hope all is well.

I am trying to get and output as per requested.

Unfortunately i am not being able to get it replicated on Alteryx, maybe because I am 100% sure doing something incorrect :)

Could i please have some guidance on this?

Please see attached my Workflow as it is today.
I am also attaching the input file.

Or this is something that we cannot do in alteryx?

As a newbie, i am still trying to understand it :)
