Hi all,
I apologize in advance because I'm sure this has been asked before; I just can't find the thread.
I've attached an example of what data I have and what I'm hoping to generate.
I'm essentially trying to cross tab (?) all records from the same person into a single row. Likely I'm overthinking what I need to do here, but nothing I've tried has worked.

I had to get creative to keep the fields in the same order but this should get you to the results you are looking for:
