Need help with a workflow - Unsure of the functions to use

Very new to Alteryx and need help with complex data set and need to get to a desired output. 

Attached is the workflow & screenshots of Input and desired Output.

Can someone please help me with this? I am lost and unsure what functionality I should use.

EntityId 724599 (from example) has two attributes with Right_Mandatory = 0.
Can you explain how it should work?
