Merge Record and rename columns

Hi! I need to merge these records based on unique id and rename column names accordingly.

 

Also, I need the data to enter a new row if the no of names > 3. Thanks!

 

The first table is the input and the second table is the desired output.

@Newtoalteryx based on the ask of the join here you go, but it looks like you might need a tranpose could get you from one to another.

 

Also for future reference desired output/outcome can help in getting your issue resolved.

 

You can rename columns in the join tool or add another select and do it there
