Good Afternoon Alteryx Community,
First thank you, @fmvizcaino and @afv2688 for your help on my last problem and providing a workspace solution! Your help has been extraordinary.
I am excited to have yet another question to further our research with alteryx before my final meeting Wednesday. I am looking for alteryx to look through all the preop through follow-up data and replace it with a certain value. I will post the data and then I will post the conversions for the find and replace:

I would like alteryx to go through each column and find and replace with these values so i can achieve my final calculation:

I would greatly appreciate if you would include a workspace since I believe this will be a rather complicated process. I always am grateful for your workspaces since they help me learn as well as reduce the possibility of error when recreating. Thank you again and look forward to your help,

I'm attaching a solution to your problem. The dynamic rename is being used to correct the column names.
