Load gdb or shp file in alteryx

If I include a shp file in the input data model of Alteryx I get the following message below. Could anyone tell me what I can do to solve this?

Error: Input Data (76): No available conversion between projections.
Source is:

Can you please give a little more info on  how did you form this, is there template for this that I can use. I am running in to the exact same problem.

I tried your solution and although it is not throwing the error anymore but its still not correct. I had the exact same projection file as was in question and I applied the conversion from your answer but the projection is off a bit, Please see the image below. Can you suggest what might be going on?
