Layout to be modified of dataset

I have an input data shown in screenshot as prototype and required output also mentioned in the same. Can someone please help me on this? 
P.S - Ignore the color. Its just for understanding purpose.

Try transposing the data and then removing dupes:

Hi @Luke_C Thanks for the solution,it really worked well but if in case data comes with two amount columns? I have tried with Cross tab & transpose but it didn't go well. Can you suggest for the same, that would be really appreciated?

The same solution would work, just would need to update the formula and transpose tool:
