How to manage the info input from PDF or scan copy to output list in excel

I am trying to use Computer Vision tools, recognize the data (characters, figures) in PDF and convert the data to excel.  

 

The data in red blocks should be fixed items' name, the data in purple blocks should be variant with each PDF or scan copies. If i want to have list of 12345, how can i manage that?
