How to count the record with fields as criteria

I have the input example mentioned below, I need to know how can I reach the output. Please help me out.

Can you explain a little more, how you get to your output?

Why doesn't,

Item_1	Item_2	CountDistinct_Keys
H	A	1

for example, appear in you output?

Because I have both H & A included in "626" order key

Yeah, that's also possible, I forgot to include that in my output. 

Basically I need a list of all possible combinations and count of distinct keys of those respective combinations
