How can I make check boxes next to fields in PDF outputs?

I am making PDF files through the rendering tool and the format required for these documents include checkboxes next to items and questions. How do I create these checkboxes and reflect whether they should be checked or unchecked? I see the Unicode way but I can't anticipate how clean the document will look using that.
