Format specific file

Hi all,

I have a file coming to me from a process that I cannot control.

It comes in a tabular format and I am supposed to re-format it in Alteryx and then feed the output into tableau for presentation purposes.

I have tried quite a bit however, I am unable to quite get it right. The incoming format is as per the input file and the desired output is as per the output file

Help would be much needed.
