Date, edate, 5 years multiple formula

Hi. Hello

I have a data of employee effective date.
I want to have a field where month and day is the same, but the year is changing refers to the header column.
And another field with condition:
When service year is less than 12 months, count the month of effective date to current month.
Every 5 years and multiple is 24, others than that is 12.
