Data transposed multi columns, grouped by Name

Trying to get my data transposed to columns, grouped by Name, for up to 30 "sets" or as many as it finds (set being Old+New). I have downloaded several other transpose workflows and none are quite catching what I need. Any help is greatly appreciated!
