Conversion of Dates to Quarter

Hi All,

I am trying to convert the dates in my data set to quarter. Created a formula based on various solutions provided in discussion group and am not getting the desired output. Can somebody have a look and let me know where I am going wrong.

Data set and Workflow is attached for reference.

you formula looks very unclear to me, therefore I went with a clean approach:
