Consolidate data to produce date

Hi Community,

Currently i would like to format the date after extracting the data (by using text to column) from the file. The date format as belows. May i know how can i build the workflow to combine the data as below to produce the desired result.

I have prepared a workflow for you:

First I have prepared data and afterwards, I have used the date time parse function to create proper output.
