Combine / Concat Two or more column and create new one added to the file

I am having difficulties to Concat two or more columns into one and add that column to the original file.

Column A       Column B        Column C       Column D

Bob                Smith               Designer         Bob Smith Designer

John              Moor                Player             John More Player

Sam              coper                Writter            Sam Coper W

...

...

Please help.
