Can not request a free trail both in web site and designer software

I first tried to submit the request form on web page. It shows the error message "This field is required", but I filled all the items and check box above.

Then I tried to download the designer and submit the request in the software, it shows the error message as below:

 I have tried both of my business email for free trail and my personal email for 'Spark ED'. None of them receive any response email yet.
