Arranging vertical data to a horizontal table

I have a requirement of arranging data in the vertical multi row data to a simple horizontal table. Any advice on how to achieve this is greatly appreciated.

LastName
Ph.num	1234567890
zip code
City	Phoenix	Spouse FirstName
Country	US	Spouse LastName
Address

to 

FirstName	LastName	Ph.num	zip code	City	Country	Address	Spouse FirstName	Spouse LastName
		1234567890		Phoenix	US
