App for Filter Tool

Hello Friends,

I have attached a excel file with Input and Output details. I need your help to create and app. You will find a column name MONTH with single digits in it. Those are months. 1 means Jan, 2 means Feb and 12 is? You are right Dec.

Now I want a dropdown or any other option on an app so that if I want output for the month June then I should type 6 and workflow should give me detail of the month 6.

Let me know for any other detail.
