App States Formula doesn't recognize column but it does in desktop

I'm having an issue where my app runs fine in desktop however, when running as an app in the gallery or through a desktop app interface, the formula doesn't recognize the columns.  Does any know what's happening? 

Below is how the interface should be checked after the example file is uploaded (there won't be as many column choices using the example file)

Change the action type on the select tool to be "Update select with multi-select Listbox". That fixed it for me when I tested your workflow.
