All Combinations Two Columns

Need to a way to find all combinations using two columns.  See the attached data set.  I've included what the input looks like in tab 1 along with what the desired output would look like in tab 2.  Basically for each store I want every possible combination of geo.

How did you determine the "comboId" field in your output list? 

What about the duplicates?  2 stores X 5 Geos only gives 10 combinations.  Where do the extras in your output come from?
