How to assign the SCORE to a list of values, based on these score conditions?

Hello, I am trying to create a workflow in Alteryx by which I can Assign the SCORE to a list of values, based on the score conditions.

I have attached the sample data in an excel sheet.


On the top there is the table to score my value.
I need to score the items according to the Category and the Value

Example:

Items A
Category - 2
Value - 1606
Score - 3

Item B
Category - 3
Value - 1000
Score - 3

Please suggest me, which particular Alteryx Tools should be used in this workflow for getting the required solution. If you need any more clarifications, then do let me know.
