Lookup value based on separate input

I have template file that wants to populate an amount based on date and need to lookup the values from another input, however the date criteria comes from another input.  

First text input has data

Second input has the blank template where values need to be added

Third text input that will be entered and determines what column and line the amounts will fall on the Second input

This workflow will be used daily, so that is why I need the flexibility to enter the current data for looking up values
