Transposing the data

Dear All,

I am stuck at a point and need your advice or sample workflow for the issue please,

I am trying to transpose the data from Jan 19 to Dec 19 and i also need to get a seperate column with Grade which is a row above the month.

Output columns required are  -

A | B | C | D | E | F | G | H | I | J | K | L | Months | Grade

Please help me with this, thank you so much in advance.
