How to fill in nulls w/data from row below-Same Data but different values in two fields

Hi There!

Below is a screenshot of my workflow. The second screen shot is what I am trying to achieve but have failed to do so. Please help?

How do I get the values from "NAME NO" filled in to the nulls and same with "NAME YES".

I am trying to consolidate the rows because all of the data are the same and I am trying to have just one row by filling in the nulls with the value below.
