Create a column per record containing the file and tab names

Hi,

I have a single workbook with multiple tabs and I want to add the file name and tab name as a file name to the end of each field.

So my data to work with looks like this:

I have tried a CrewMacro and I have also tried Dynamic input, so far I have been unable to achieve the desired result!

Any ideas
