two headers and transpose

Hello Everyone,

Need to convert two header file to transpose and then prepare an analysis file.

I have highlighted the red one as original file, yellow is the desired format. Can you help me here.

I used dynamic rename but getting only one header, I need to include the 1st one as well and then transpose all

Maybe the attached workflow will help you, to get closer to your solution. 

If it's not good for you, then sorry This is the most i can do right now.

I hope it's not so stupid, i'm almost never use transpose tool.
