Formatting Rows and Columns on Report

Hi, I am looking for a way to format data into a specific format as I have attached here.

I have a file in the given format which I have added a custom date header.

 

I want the workflow to export a report in the given format as an excel file. I tried various approach using Generate Rows, Multi-Row Formula but couldn't get to this format. 

Any suggestions are much appreciated!
