Convert Single Column to Multiple Columns

Hi,

Trying to convert a single column to multiple columns using the Cross Tab tool, but having trouble getting to the desired result.

Each value in the Description column needs to be converted into a separate column (each Description value ends up becoming a column header).

Any help would be greatly appreciated!
