Combining Columns or Formatting Data

Hello,

I am looking to combine columns into one single output field. There are dashes I need to include as well. I feel like I am overcomplicating it but the desired output is in the format below.

I figured it out. I used a formula to bring the fields together. I forgot that I could simply use a + to combine string fields in a formula.
