Can you please share the screenshot of the Output tool configuration.

Maybe your Field [FileName] only have full path for file name, but have not sheet name or range ?
Please remove the highlighted from your Output tool.
The below setting will work perfectly, I have tried in my desktop.

Hey everyone getting this issue... i would note 1 thing that i keep seeing happening..
STRING LENGTH
make sure your filename / file in the output is complete, i had limited of 100 set when creating the name in the formula tool which broke the filename
