Cross tab for multiple columns. (Need to keep count and ammount with category)

Hello,

I can't quite figure this out.  I'm trying to use Cross tab for multiple columns. (Need to keep count and ammount with category).  I've attached my sample data as welll as screenshots of my workflow and the sample data updated layout.
